1. Working as part of a multidisciplinary team, the HCA will ensure the highest standard of care is maintained to each individual patient
ACCOUNTABILITY
2. Accountable to OC Nursing
PRINCIPLE TASKS
3. To undertake nursing and administrative duties as instructed by Ward Manger or shift coordinator.
4. Adhere to unit policies and protocols reading ward and neuro clinical guidelines every 6 months
5. Ensure practice is conducted inline with NMC recommendations
6. Participate in continuous professional development activities that improve your clinical effectiveness
7. Undertake initial and yearly Health and Safety, fire, ILS and manual handling briefs.
8. Work within unit clinical risk management guidelines. Read unit clinical governance policy bi annually
9. When allocated patients to a given patient you will be responsible for delivering and evaluating the patients all aspects of the patients care during your span of duty and reporting any areas beyond your remit to the nurse co-ordinating the shift All care is provided according to professional guidelines on accountability
10. Assist in dietary needs of patient including ordering meals, servicing meals on the ward ,monitoring standards of food and feeding disabled patients
11. Complete all observations and behavioural charts directed
12. Escort patients to hospital appointments requirements as required
13. Ensure all patients allocated to your care attend all therapy sessions identified on their daily dairy escorting them to sessions if required
14. Observe therapy sessions at regular intervals on named patients and feedback progress and directives to nursing team
15. Maintain effective communication with verbal handover of all allocated patients to other nursing team members at middle and end of shift.
16. Be aware of unit code blue procedures.
17. Identify all hazards and risks within the working environment. Report Health and safety hazards to SHEF representative at the earliest opportunity
18. Complete clinical incident forms at earliest opportunity in line with unit policy for all clinical incidents and near misses.Shifts to include Early (E) 7.5, Late (L) 6.5 hours,12hr Day (12) and Night (N) 12 Hrs
