An opportunity has arisen for a full time Case Manager to join the Shepway Coordination Team based in Queen’s House in Folkestone.
Having a professional qualification in a related d! iscipline e.g. Social Work, Nursing (RGN level), or Occupational Therapy is essential.
We are looking for someone with initiative who is able to use a person centred approach to support Service Users in the assessment of their eligible needs, assist in support planning and monitor / review their personal outcomes. You should be committed to working in partnership with service users, carers, and partner and provider agencies to deliver both the personalisation and health and social care integration agenda.
The post offers an excellent opportunity for career development and you will be afforded supportive management and regular supervision.
The ability to travel across a wide geographical area in a timely and flexible manner at various times of the day is essential.
