We are looking to recruit into the following Clinical Nurse Specialist (CNS) positions at our Sydenham and Orpington sites:
CNS (BAND 7) CARE CO-ORDINATION CENTRE
Full or Part-time
Location: Orpington, Kent
The St Christopher’s Group has been successful in a bid to become a Care Coordination Centre for all people in their last year of life in Bromley. This exciting and innovative service will enable people who are not currently referred to our specialist palliative care services to have their care co-ordinated by the new team. It is anticipated that many patients will have a non- malignancy, such as dementia, heart failure, COPD and will not require the involvement of the hospice palliative care team.
The CNS will work as part of the care ! co-ordination team and will work in partnership with Bromley H! ealth care, GPs and local voluntary organisations. The post-holder will carry out initial assessments, where possible in joint visits with district nurses or community matrons and will ensure that there is an opportunity for advance care planning.

CNS (BAND 7) HOMECARE
Full or Part-time
Location: Sydenham or Orpington
The St Christopher’s Group has 40 CNSs divided into 5 teams, 2 based at Harris HospisCare in Orpington and the other 3 at St Christopher’s in Sydenham. Our CNSs carry a caseload of 20-25 patients, 25% of whom are referred with a nonmalignant condition. CNSs assess patients and families both at home and in our outpatient clinic centre. We offer a 24/7 advice and visiting service and work closely with our wider multi-disciplinary team.

PRACTICE DEVELOPMENT/CNS FOR CARE HOMES (BAND 7)
Full or Part-time
Location: Sydenham
This is ! a role within our Care Home Project Team which works with nursing care homes implementing the Gold Standards Framework (GSF). The team also offers specialist support for those patients in care homes presenting with complex needs.

Applicants will have proven palliative care/oncology experience with relevant qualifi cations and signifi cant experience of working as an RGN at Band 6 or above. Applicants must also possess a current driving licence. Applicants for the CNS Band 7 Care Co-ordination Centre role will have proven palliative care/oncology or community experience in long term conditions with relevant qualifications and experience of working as a Band 7. Post-holders will also be part of the out of hours on call rota.
We are keen to develop our CNSs into advanced practitioners and our team delivers an advanced assessment/clinical examination module in partnership with King’s College London. We also support CNSs in becomin! g independent prescribers.
CNSs benefit from our lease car scheme, ! the use of a pool car or a car allowance depending on hours of work and location. Please note that continuity of NHS Superannuation Pension is also available.
